The Process is Simple!

Just follow the 4 easy steps to start the process to obtain your Medical Marijuana Card in Massachusetts


At Massachusetts Marijuana Doctors we’ve made the medical marijuana evaluation and certification process as simple as possible for you. We guide you through each step so you’re never on your own and we answer all questions you have along the way.

Upon evaluation with our medical marijuana doctor, if you qualify for medical marijuana treatment you will receive a physician certification form for application to the Massachusetts Department of Public Health for your medical marijuana identification card. We will assist you with the submittal of your medical marijuana identification card application upon request. To get started follow the simple steps below now.

Step 1: Qualifying Conditions

The first step before your evaluation is to verify you have a debilitating medical condition that qualifies you for medical marijuana treatment under the Massachusetts Medical Marijuana Program statutes.
Debilitating medical condition” means one or more of the following:

  • Cancer
  • Glaucoma
  • Positive status for human immunodeficiency virus
  • Acquired immune deficiency syndrome (AIDS)
  • Hepatitis C
  • Amyotrophic lateral sclerosis (ALS)
  • Crohn’s disease
  • Parkinson’s disease
  • Multiple sclerosis
  • Other conditions as determined in writing by a qualified physician

or

  • Chronic Pain of at least 6 months duration
  • Chronic muscle spasm
  • Cachexia (wasting syndrome)
  • Agitation of Alzheimer’s disease
  • PTSD
  • Painful peripheral neuropathy
  • Terminal illness (hospice care)
  • Radiculopathy
  • Degenerative Arthritis

Step 2: Medical Records Review

In order to schedule your patient evaluation, we’ll need to review you medical records to verify you have been diagnosed with a debilitating condition approved by the Massachusetts medical marijuana program. Without your medical records our physicians are unable to provide certification required by state law.

We offer the fastest certification possible by providing you with our online medical release form, so our doctors can request your medical records from your current treatment provider(s) right away. If you have your current medical records available describing your approved debilitating condition contact us now to schedule your evaluation. Step 3: Physician Evaluation

After your medical records have been reviewed by our certified physician, and determine that your debilitating condition qualifies for the Massachusetts medical marijuana program, a member of our staff will contact you to schedule your evaluation with one of our licensed physicians.

During your Medical Marijuana Evaluation appointment, our physician will communicate with you to determine your needs and assess if medical marijuana is the right choice for you. We will discuss your qualifying condition for medical marijuana treatment and upon approval; qualified patients will receive a signed Physician Certification Form required by the Massachusetts Department of Public Health. At your evaluation you must present a valid Arizona ID, such as a Massachusetts ID or Massachusetts driver’s license, along with the physician visit fee.
Insurance companies do not cover medical marijuana consultations or the state’s medical marijuana application fee.

Step 4: Medical Marijuana Identification Card Application

Once you receive your physician certification for medical marijuana we can assist you in applying for your medical marijuana identification card with the Massachusetts Department of Public Health.
